The symptoms you describe could be caused by a variety of ailments. We'd suggest inspecting anad cleaning the EGR (exhaust gas recirculation) and PCV (crankcase verntilation) systems, and have the exhaust system checked-out for blockages. DO NOT remove the EGR if it is blocked, because it serves an important role in reducing combustion- and exhaust temperatures, just clean it and refit it.
Another possibility is a boost leak between the turbo and intake manifold, which could be caused by a cracked intercooler or deteriorated boost pipes. In short, a capable diesel specialist should be able to pinpoint the problem, especially if you supply them with this list of possible causes.
